How to Complete the Organ Transport Box Puzzle in Resident Evil Requiem

In Resident Evil Requiem, you’ll encounter various challenging puzzles, including the need to find specific items that trigger further actions—like opening doors or progressing through the game. Capcom’s design includes intricate horror mazes that often send you chasing after items across confusing layouts.

One of these puzzles involves the Organ Transport Box. To move forward, you must unlock this box to retrieve an artificial organ. You’ll then need to insert this organ into a deceased body to obtain a crucial keycard. Essentially, you’re collecting one item to unlock another so you can continue exploring Rhodes Hill Critical Care Center.

Where to Find the Organ Transport Box

The Organ Transport Box is located in the garage of Rhodes Hill Critical Care Center. It’s inside an open freezer van. The box waits to be picked up, but beware: as soon as you do, a zombie infected with the virus will crash through the wall in a construction vehicle, heading your way.

To avoid getting killed, aim for the zombie’s head immediately. It only takes one shot to bring him down and stop the vehicle. Afterward, you can squeeze through the wreckage and return inside to continue.

How to Open the Organ Transport Box

You won’t be able to open the box until you receive specific instructions. These instructions are stored in the safe in the Director’s Office, which also holds one of three quartz stones needed to escape the facility later. Next to the safe, there’s a red binder containing the instructions for opening the box.

Open the binder, and inside you’ll find a piece of paper with the steps to unlock the box and collect its contents. Keep in mind, everything must be done in a precise order; otherwise, the process resets.

Note: Even if you know the code, it won’t work until you’ve first received the instructions from the Director’s Office. To do that, you need to grab the corrosive acid from the room opposite the safe to access it.

The Setup for Opening the Box

The image displaying the instructions shows the necessary configuration to unlock the box — don’t forget to set the circular switch to ‘Auto’ as the final step. The process involves:

Ensure the circular switch is set to ‘Auto’ then toggle it to ‘Hand’.
Move the right-hand switch from the bottom to the middle position.
Position the five individual buttons according to the instructions.
Switch the circular dial back to ‘Auto’, which will automatically open the box.

Inserting the Organs and Getting the Keycard

After opening the box, you can return to the medical examination room. Here, insert both organs into the body of the coma patient, which will then come back to life with a zombie wearing a Level 2 wristband keycard. You’ll need to eliminate this zombie to obtain the keycard and further your progress.
