Knee osteoarthritis is one of the most prevalent joint conditions worldwide, especially as people age. It occurs when the smooth cartilage cushioning the ends of bones gradually wears away. Without this protective layer, the bones grind against each other, leading to pain, stiffness, and swelling. Over time, simple activities like walking or climbing stairs can become increasingly difficult.

Many believe that rest is the best remedy for knee pain, but medical professionals have known for years that staying active is actually crucial. Exercise keeps joints mobile, strengthens the muscles surrounding the knee, and boosts overall health. However, for a long time, there was uncertainty about which forms of exercise are most effective.

A recent comprehensive study published in The BMJ has shed new light. Researchers analyzed 217 clinical trials conducted between 1990 and 2024, involving over 15,000 individuals with knee osteoarthritis. The aim was to compare different exercise types to determine which provided the greatest benefits.

The study looked at various forms of physical activity, including aerobic exercises like walking, cycling, and swimming, as well as strength training, stretching routines, balance exercises, and mind-body practices such as yoga. Researchers carefully tracked how each activity impacted pain, joint movement, walking ability, and overall quality of life over different periods.

Findings revealed that aerobic exercise was the most effective overall. Participants who engaged in regular aerobic activity experienced reduced pain and improved mobility both in the short and long term. They also reported easier walking and a higher quality of life.

Other exercise types still offered benefits but were not as impactful as aerobic workouts. Strength training was helpful for enhancing joint function over time, while mind-body practices like yoga provided short-term relief. Combining these methods with aerobic activity was shown to be most effective.

An important takeaway is that exercise is safe. None of the activity types caused significant harm compared to those who did not exercise. This is notable, as many individuals with knee pain worry that movement might worsen their condition.

The study authors acknowledged some limitations. Not all trials were of the same quality, and some comparisons were indirect. Additionally, there was limited data on long-term effects beyond a few years. Nonetheless, given the large number of participants and studies, these results are considered trustworthy.

In simple terms, this research suggests that gentle, consistent movement is one of the best treatments for knee osteoarthritis. Activities like walking or swimming are not only safe but also highly effective at reducing discomfort and improving daily functioning.

Practically speaking, aerobic exercise enhances blood circulation, strengthens muscles, keeps joints flexible, and helps manage weight—all factors that relieve pressure on the knees.

Overall, the evidence strongly supports aerobic activity as the primary strategy for managing knee osteoarthritis. Other exercises can complement this approach but should not replace it. While knee osteoarthritis may not be entirely curable, it can be effectively managed through regular activity. Simple routines, like walking, can significantly lessen pain and improve quality of life.

Additional long-term research is still needed, but the core message is clear: staying active is one of the most beneficial actions you can take for your knees.
