- Complete various runs of the Elden Ring: Nightreign Tricephalos Expedition as any character until you obtain The Old Pocketwatch item.
- Return to the Roundtable Hold and talk to the Priestess NPC dressed in white.
- Choose to give her the Pocketwatch.
- A brief cutscene will play, introducing the Duchess.
- From that point on, the Duchess will be permanently available as a character selection option.
It’s good to note that you keep the Pocketwatch even after showing it to the Priestess, so you don’t have to worry about losing it.
