Rare Vision Loss Disease Tied to Common Diabetes Medication

A popular medication for diabetes management is now under scrutiny after new research indicates it may increase the risk of a rare but serious eye condition that can result in permanent vision loss.

Researchers from the University of Southern Denmark conducted two extensive studies examining the safety profile of Ozempic, a widely prescribed drug for type 2 diabetes. This medication has surged in popularity recently because it not only helps regulate blood sugar levels but also supports weight reduction. Nonetheless, recent findings highlight a potential unexpected side effect related to eye health.

The concern centers around non-arteritic anterior ischemic optic neuropathy (NAION), a condition where blood flow to the optic nerve suddenly diminishes. The optic nerve is crucial for transmitting visual signals to the brain. When it sustains damage, it can lead to rapid vision loss, which might be irreversible.

Though NAION is uncommon, its impact can be devastating. Individuals affected may experience vision loss in one eye, and in some cases, the other eye might become involved later on. Because even a slight increase in risk carries serious implications, health professionals and researchers are paying close attention.

This issue was initially brought up by a smaller U.S. study that suggested Ozempic could more than double the likelihood of developing NAION. The newer studies from Denmark bolster this evidence thanks to larger patient populations and the utilization of national health data.

One of the Danish studies analyzed medical records from over 424,000 individuals with type 2 diabetes, including more than 106,000 who used Ozempic. The results indicated that patients on Ozempic had approximately twice the risk of developing NAION when compared to those on other diabetes therapies.

The researchers also observed a trend over time. Before Ozempic’s widespread adoption in Denmark around 2018, annual NAION cases ranged from 60 to 70. Recently, this number rose to roughly 150 cases per year, predominantly among those with type 2 diabetes.

A second study focused on new users of Ozempic, comparing their health outcomes with those starting other diabetes treatments in Denmark and Norway. This approach allowed for a more precise comparison between similar groups. The findings were consistent, again showing that Ozempic users faced about double the risk of NAION.

While these findings may seem alarming, it’s important to emphasize that the overall risk remains low. NAION remains a rare condition, and most patients taking Ozempic will never experience it. For many, the benefits of controlling blood sugar and reducing the risk of complications like cardiovascular and renal issues outweigh the potential risks.

Experts advise that treatment decisions should always weigh both benefits and harms. Managing type 2 diabetes effectively is critical because high blood sugar can contribute to serious issues such as heart disease, kidney failure, and nerve damage.

Healthcare providers also note that alternative medications are available, many of which can offer similar benefits in blood sugar control and organ protection. For patients with existing vision problems or higher susceptibility to eye issues, discussing these options with a healthcare professional might be advisable.

Patients currently on Ozempic should not discontinue use without medical guidance. Abruptly stopping treatment may lead to poor blood sugar regulation and other health complications. Instead, anyone concerned about side effects should consult their doctor, particularly if they notice sudden changes in vision, such as blurriness or dark spots.

The findings from these studies have been shared with health authorities and may influence future guidelines on Ozempic’s safe use. Researchers hope this information will facilitate more individualized treatment plans, enabling doctors to select the most appropriate medication based on each patient’s unique risks.

The studies were published in the International Journal of Retina and Vitreous and are also available as preprints on medRxiv. While more research is necessary, these results underscore the importance of ongoing monitoring for side effects and continuous evaluation of widely used medications.
