One of the main issues you and your companions face in Baldur’s Gate 3 is contracting a Mind Flayer Parasite. Throughout the game, you’ll encounter various quests aiming to cure you of this illness. One of the earliest quests involves Lae’zel trying to locate the nearest Githyanki Creche.
You can find the Githyanki Creche along the Mountain Pass. After reaching the Rosymorn Monastery, you’ll come across a Githyanki in the infirmary who is studying Mind Flayer parasites and wants to use the Zaith’isk device to attempt a cure. Keep in mind that if you previously allowed Volo to try curing you, you might already be aware that these procedures can have unexpected consequences.
Purifying yourself of the Illithid parasite is a top priority in Baldur’s Gate 3. Your party will frequently try different methods to eliminate it. While none are guaranteed to work, they often result in temporary buffs or debuffs. It’s important to know what to expect from each attempt. This guide has been updated with all relevant information after the latest game patch.
Should You Tell Ghustil That You Are Infected?
When you reach the Creche, you’ll be free to explore and talk to the various inhabitants. One of the first characters you’ll meet is Ghustil, a Githyanki working in the Creche’s Infirmary. If you haven’t already, it’s best to have Lae’zel in your party for this part of the story.
You’ll also be here to speak with Vlaakith and present the artifact, but it’s important to do the infirmary tasks before that. Speaking with Vlaakith first will make the entire Creche hostile and prevent access to the Zaith’isk device. Finishing the infirmary quest first may trigger a small fight, but it doesn’t extend to the rest of the Creche. Additionally, doing the scene with Ghustil beforehand makes it easier to convince Lae’zel to stand down if you refuse Vlaakith’s orders. Otherwise, you’ll need a high check to persuade Lae’zel to follow your lead.
When discussing the parasite with Ghustil, you can choose to admit you’re infected or claim a friend is. If you lie about having a friend, you’ll be forced to admit your own infection to proceed. Ghustil will not get upset if you admit your infection and will agree to use the Zaith’isk device to cure you. However, Lae’zel will insist on using it first.
Should You Let Lae’zel Use the Zaith’isk First?
Lae’zel’s approval will decrease if you deny her the chance to use the Zaith’isk. Conversely, you’ll gain her approval if you let her go first, as this is what she’s been eager for since you met her. If you want to see the outcomes of both options, it’s best to save your game, let her go first, then reload and choose to go first yourself.

Ultimately, it doesn’t matter who enters the Zaith’isk first, as the device causes whoever is inside to start dying. A strange entity, called the Dream Visitor, will warn you to stop. At this point, you must succeed in several skill checks before proceeding. Saving before attempting is highly recommended because the saves are difficult.
How to Pass All Saving Throws in the Zaith’isk
The required Saving Throws to resist damage from the Zaith’isk are intentionally tough. Before entering, ensure your character has equipped gear or items that boost relevant stats. During your time inside, you’ll need to make difficult checks to convince yourself or Lae’isk to exit.
- You’ll need to succeed on a 30 Deception or Persuasion check to convince either yourself or Lae’isk to leave.
- Successfully convincing Lae’isk to exit will grant you a +1 Approval boost from everyone.
Encouraging Lae’isk or succeeding in skill checks while inside will increase her approval. The stats needed to succeed on the saving throws include:
- 12 Intelligence
- 15 Wisdom
- 18 Constitution (or other values depending on your class)
If you manage to withstand the Zaith’isk, you will gain a permanent “Awakened” buff. This allows you to use any of your Illithid Powers as a bonus action from that point onward.
How to Increase Your Odds of Success
Since these checks rely on dice rolls, there’s no guarantee of success. Improving your chances involves increasing your character’s stats with items like Auntie Ethel’s Hair, which grants +1 to any ability score permanently. Other armor and accessories, such as amulets, can boost ability scores temporarily. Check your inventory and camp supplies for these useful items, and swap them in and out as needed.
What If You Fail the Zaith’isk?
Failing the saving throws causes whoever was inside to lose -1 to all ability scores used in the checks. This penalty can be reversed by consuming a Mind Flayer sample, but this isn’t a desirable outcome. To improve your chances, make sure your character’s stats are as high as possible and save your game beforehand.
Regardless of success or failure, the Dream Visitor will intervene to prevent death and will pull you or Lae’isk out of the device. Ghustil will be upset that her machine is broken but will still want to extract the parasite.
Should You Lie to Ghustil?
Once you exit the Zaith’isk, you can try convincing Ghustil that it worked or argue with her. If you cannot persuade her it was successful, she will leave angrily and lock the door behind her. You can attempt to lockpick the door to follow her, but if you do not leave immediately, she will return with backup, leading to a fight with the entire infirmary staff. During this fight, you will need to kill her and some young Githyanki.
The rest of the Creche will not turn hostile after this fight, so you’re free to explore and speak with other characters afterward. Following this, Lae’zel will want you to speak with the captain about tampering with the Zaith’isk, but you won’t be able to discuss it with Kith’rak. From there, you can continue with other tasks, such as taking a Githyanki Egg, which you can convince Lae’zel to raise, or speaking with Vlaakith.
