The last glitch is in the fourth level of World 5, called Crumbled Canyon. At the very start, look for a large metal pipe on the left side. Climb it by jumping onto its metal sides, then head toward the wall with the final glitch. Reaching this spot unlocks Glitch Boy if you haven’t already collected the past ones.
