Starting on the fourth day of Pathologic 3, your main task is diagnosing patients with secondary illnesses—conditions that slow down the deadly Sand Pest. Getting accurate diagnoses is crucial: correctly identifying each disease helps you unlock a vaccine the next day, which greatly lowers infection rates and allows you to advance in the game.
We’ll guide you through the diagnosis steps, covering how to collect symptoms, analyze biological samples, and carry out investigations outside the hospital. There’s also a complete list of patient diagnoses for those who need help or prefer to focus on other game parts.

How to Diagnose Patients
To successfully diagnose diseases:

Start with detailed symptom collection. Examine each patient closely by checking their dialogue to see if they mention or display visible signs like facial discolouration or skin lesions. Use bodily examinations to spot symptoms. After noting potential signs in your casebook, confirm them by discussing with the patient—if selecting certain symptoms unlocks more dialogue options, those are real indicators worth noting.

Reduce the possibilities. Cross-check all gathered symptoms against known diseases. If only one disease fits, you’ve likely found the correct one. Often, you’ll need to choose between a few options.

Use lab samples for tricky cases. When symptoms overlap between two diseases, biological samples become vital. Focus on which organs are infected or healthy based on the disease profiles. The info panel for each disease shows which organs are affected when the patient has it.

Investigate outside the hospital when needed. Some conditions require field investigations. Your patient’s dialogue will hint when this is necessary, and a marker will appear on your map indicating where to explore. These investigations can reveal additional symptoms or help you eliminate wrong options.
